Oregon Psilocybin Services (OPS) has received feedback from licensees about the need for additional forms to meet OPS requirements in administrative rules and to help clarify expectations.

Oregon Psilocybin Services (OPS) has received feedback from licensees about the need for additional forms to meet OPS requirements in administrative rules and to help clarify expectations. In response to this, OPS has created new operational forms and updated some existing forms for licensees. These forms help OPS licensees comply with OPS administrative rules.

You can now find all updated OPS operational forms on the Facilitator License or Service Center License web pages. Client forms may also be found on the Access Psilocybin Services web page. Spanish versions of the new and updated forms are currently being translated and will also be posted as soon as they are available.

Please consult the rules to determine whether a form must be completed for every client or whether it must be completed only when certain conditions or events occur.
